Medical Day Spa was born from an idea to offer thoughtful, high-quality aesthetic care in a space that feels welcoming and grounded in medical expertise.
While operating their own surgery center, Dr. Berger and Julia spent years searching across the Triangle for aesthetic/spa treatments that met the same high standards they held themselves and their team to — they were always left wanting more. Eventually, they asked themselves a simple question: Why not do it ourselves? With that, Medical Day Spa was born — a place where the highest quality care could be provided to friends, family, and anyone seeking advanced, safe, and effective aesthetic treatments.
What started as a passion project, across the street from the surgery center, quickly evolved into something more. On their days off and after surgery hours, the team began seeing patients, learning new techniques, and discovering how much joy they found in helping people feel confident in their skin. They travelled around the country learning from pioneers in the aesthetic space and bringing that knowledge back to Chapel Hill. Some of those early team members are still with us today — bringing the same heart and dedication they always have.
In 2019, the search began for a permanent home — a space that reflected the beauty, intention, and quality behind our work. We found it in the heart of Southern Village, Chapel Hill, and opened our doors just before everything changed in 2020. Thanks to a strong foundation and a deeply committed team, MDS was able to stay open, retain every staff member, and continue serving the community through it all.
Since then, Medical Day Spa has grown — not just in size, but in purpose. We've become a trusted part of the Triangle community, known for honest care, skilled providers, and a focus on natural results.
Hillary Casselli, RN, one of our first injectors and the first provider hired from outside the surgery center, took over ownership from Dr. Berger and Julia in 2024. She is a highly sought after injector with years of experience, and patients drive from all over North Carolina to see her. She leads our team alongside Dr. Otter, our Medical Director and Rachael Taylor PA-C, our Clinical Director.
Dr. Otter brings a unique and heartfelt focus to women’s health — particularly supporting patients through life’s transformative seasons such as postpartum, perimenopause, and menopause. Her goal is to provide thoughtful, compassionate care that fills the gaps too often left in traditional healthcare.
Rachael did orthopedic surgery for 13 years, practicing with some of the leading minds in the field, before making the transition to aesthetics. Hungry to understand how everything works, she is our guru on all things aesthetics and deep dives into every clinical study to ensure we offer the best.
With years of experience and a passion for subtle, confidence-boosting results, Hillary, Dr. Otter, and Rachael carry forward the values that started it all.
